Patients are requested to bring insurance cards at every visit; a list of all oral medications (including over the counter medications); and a list of all medications/creams used for the current problem. If any biopsies have been performed relevant to the current problem, please bring along a copy of the biopsy report. All patients should arrive 15 minutes prior to their appointment time for paperwork.
Payment Options/Accepted Insurance Plans
Dr. Robinson participates in several PPO insurance plans as noted: Medicare, Humana - as Medicare non-HMO Replacement policies only}, Aetna, United Healthcare, Health Choice, BCBS, Preferred Community Choice, First Health, and Cigna (non-HMO). We will file claims for patients in these plans. It is wise to double check with our office prior to being seen to insure that we still participate in the above plans. We do not accept HMO/referral insurance plans.
Patients are responsible for payment of any copayments and surgical deductibles at the time of service. Some services may not be deemed medically necessary by your insurance company or Medicare. Charges for these services are the responsibility of the patient at the time of service.
- Insurance Cards:
Please bring your insurance card(s) with you at every visit. In addition, information on the insurance card holder must be supplied. This includes name, address, and phone number, SSN, birthdate, employer, employer address and phone number. If the patient is under 18 years of age, the parent or legal guardian must be present for the first visit.
If you are unable to keep your appointment, please give our office at least a 24 hour notice. This will enable another patient to use that appointment time and avoid a cancellation charge.
- Office Courtesy
Please be aware that a certain amount of time has been allotted for each patient. We try to maintain an even and on-time office flow, but occasionally emergencies arise. Office flow is smoother if patients arrive several minutes early to check in. If you bring other family members with you, please limit comments and questions pertinent to the scheduled patient only as a courtesy to the patients waiting their turn to see the doctor.
Your allotted office visit time may not allow for numerous surgical procedures to be performed. In addition, insurance companies limit the number of procedures that can be performed in one visit. Rescheduling may need to be done for certain surgical procedures.
